What is the reunite service? |
|
Once an adoption process has been completed, there are instances where families from either side loose contact due to different social values, customs, personal values, etc. Obviously for the adoptee, when knowing he/she was adopted may show strong interest to his/her heritage background or hopes to contact his/her biological parents. In such case, he/she can get in touch with Adoption Information Center and request to be reunited with his/her biological parents and vice versa for parents who gave up their child for adoption. They can too, contact Adoption Information Center to arrange a meeting to be reunited with their adopted child.

The information will only be provided once his/her (adoptee) identity has been confirmed. A written and signed letter of consent by legal guardian is required if he/she (adoptee) is underage. |
| 2. |
Adoptee's legal guardian(s) or birth parent(s), adopting parents and person of interest: |

Once identities have been confirmed, and adoptee or adopting parents have signed an information release form or provide an agreement afterwards. In cases where adopting parents or adoptees have gone missing or have passed that have not signed such agreement, Adoption Information Management Committee will evaluate the situation and decide if any information should be passed on. |

Adoption Documentations:
Adoption documentations application can be divided into identity related and non-identity related information. |
| 1.Identity information: |
| Adoptee, adopting parents and adoptee’s legal guardian(s) or birth parents, siblings, relatives and related person must provide full name, permanent address, current address, personal ID number or any other identity identifiable material |
| 2.Non-identity information: |

Adoptee, adopting parents and adoptee’s legal guardian(s) or birth parents,siblings, relatives and related person’s work place. |

Adoption application papers. |

Adoption consent form. |

Court records. |
| |
Adopting family’s visit report. |

Application procedure: |
|
Applicants who fit in the qualifications can apply the adoption documentations through Adoption Information Center by fill in the application form and provide any necessary personal identification materials. An initial meeting or interview will be arranged by the center’s social worker to assess and evaluate the applicant’s qualifications, motivations and other specifics for the application.

Things to look out for when applying: |
| 1. |
Age restriction: Adoptee under the age of 20 must attaches a written consentletter from his/her legal guardian. |
| 2. |
Applicant restriction: Applicants who have their preliminary assessment rejectedby the Adoption Information Center can appeal against the decision and ask for a re-evaluation by Committee of Adoption Information Management. If the re-evaluation is rejected still, an appeal can be made directly to the central competent authority. |
| 3. |
Releasing identity information restriction: Identity information will only bepassed on if the person has agreed to do so. In cases where the adoptee is over 20 years of age and his/her identity has been confirmed, the information should be released regardless. |
| 4. |
Change of information but have violated the agreed contract: Family giving upfor adoption and family adopting can change/correction their will on the release of ID and contact information. But if the change/correction of information from either side has violated the adoption agreement (unless necessary and must provide explanation) without an appropriate explanation, Committee of Adoption Information Management will assess and evaluate the situation. |

Negative impact of information: If there is a chance that providing suchinformation may cause mental distress to the applicant (eg. Biological parents has history of criminal offenses), Adoption Information Center will provide counseling services before releasing such information to the applicant. |
